Advanced Practice Registered Nurses / Physician Assistants
Manages the day-to-day operations of the School-Based Health Center (SBHC), as the leader of a multidisciplinary team, to ensure that all mandated program services are provided. The Supervising Nurse Practitioner also provides patient care to a panel of students enrolled in the SBHC. This work is carried out in support of the mission and goals of the Center and the Ryan Network.
Ensure the provision of care to patients with medical problems. This includes the recognition of acute symptoms, management of emergencies, discrimination between normal and abnormal findings on physical assessments, reporting of findings, and appropriate decision making. Provide routine treatment in collaboration with physician(s) based on policies, protocols, and practice agreements.
Ensure the development and implementation of a plan of care for each patient including: additional diagnostic studies, if indicated; treatments, including prescribing medications within protocols under physician's supervision; referral to other health care providers and for social services, as appropriate; teaching, counseling, assisting patients and other significant persons to assume responsibility for self-care, prevention of illness, promotion/maintenance/restoration of health; follow-up to assure continuity of care.
Participate in regularly scheduled consultation sessions with collaborating physician to present cases and seek guidance as needed regarding the care provided to patients.
Participate in the Center’s Quality Improvement Committee and take the lead in QI and performance improvement activities specific to the SBHC as a part of monitoring and enhancing patient care and other program services.
May be assigned to other primary care clinic areas, as necessary, and when the SBHC is closed.
Remain up to date on issues pertaining to school-based health center services.
Practice accepted infection control measures in accordance with the OSHA blood borne pathogens standards, and Center policies and procedures.
Supervision of staff, including but not limited to: participation in the recruiting, and hiring of staff, as well as assessing competency and evaluating performance, and providing day-to-day supervision and support to ensure that all staff meet recognized standards for delivery of quality health care and services, professionalism and productivity. Maintenance of attendance records, staff schedules, and taking disciplinary action as needed.
Management of all aspects of service delivery and program operations, which includes Practice Management, and the development, implementation and provision of quality medical care and health education to address the needs and concerns of the student population.
Coordination and collaboration with the Center’s Patient Services and the Network’s Billing Department to maximize patient services revenue and to reduce dependence on grants.
Development of program-specific procedures and implementation of Center policies.
Ensure program compliance with regulatory and funding mandates by preparing reports and responding to inquiries regarding the program.
Develop educational curricula and new program components, and assist in seeking funding for new initiatives.
Coordinate and integrate school-based services with Ryan/Chelsea-Clinton operations and programs and relevant Ryan Network Programs and Departments.
Develop and foster relationships with school staff and other community-based organizations and participate in relevant community events and activities.
Remain current on advocacy initiatives and keep the Executive Director and Deputy Director of the Ryan/Chelsea-Clinton Community Health Center apprised of legislative developments.
Attend all meetings and/or training sessions as required.
Report any unusual findings or problems pertaining to program operations to the Deputy Director and/or the Medical Director of the Ryan/Chelsea-Clinton Community Health Center.
Perform other duties and projects as assigned.
