Advanced Practice Registered Nurses / Physician Assistants

Manage, create, deliver and track education for clinical staff and students, participate in education of health professions trainees.
Direct the process improvement process and reporting (QAPI, HIS, CAHPS, PIPs.) Work collaboratively with staff to go beyond the required measures to pursue excellence in care delivery and efficiency.
Perform Face-to-Face (F2F) examinations of patients in accordance with CMS guidelines and completes documentation timely
As needed, round on patients in the Inpatient Care Unit (IPU), providing direct patient care in collaboration with other members of the medical staff and interdisciplinary team. Conduct interviews of patients and their families, assess and discuss perception of illness and prognosis, symptom distress, work with interdisciplinary team and prescribe nonpharmacologic therapies, pharmacologic treatments and educates about treatments. Complete documentation timely. Perform symptom visits to patients in their homes on request of nurse care manager
Develop and pursue personal professional objectives in collaboration with members of the medical staff
Assume other duties and responsibilities as determined by the Medical Director that are appropriate to the skills of the practitioner.
Current licensure to practice advance practice nursing; must obtain licensure in the District of Columbia and Maryland according to established policy
Current DEA and DC and Maryland controlled substance prescribing licenses
Graduation from an American Academy of Nurse Practitioners or American Nurses Credentialing Center or accredited BSN & MSN programs
ANA certifications as adult and/or geriatric nurse practitioner
Strong preference for Certified Hospice Palliative Nurse Practitioner
Strong preference for 1 or more years of hospice experience and clear understanding of hospice regulations
QAPI and education experience preferred
Strong organizational skills, able to prioritize tasks
Available 5 days per week
Physical and emotional health sufficient to meet job demands and the criteria for the annual physical examination
