Implements and monitorspatient care plans. Monitors, records and communicates patient condition asappropriate.
Serves as a primarycoordinator of all disciplines for well-coordinated patient care.
Notes and carries outphysician and nursing orders.
Assesses and coordinatespatient's discharge planning needs with members of the healthcare team.
Assessment - An RN uses a systematic, dynamic way to collect and analyze dataabout a patient, the first step in delivering nursing care. Assessment includesnot only physiological data, but also psychological, sociocultural, spiritual,economic, and life-style factors as well.

Problem Identification -Problem identification is the nurseâs clinical judgment about the patientâsresponse to actual or potential health conditions or needs.
Planning and Outcomes â Basedon the assessment and problem identification, the nurse sets measurable andachievable short- and long-range goals in collaboration with the patient andfamily. Assessment data, problem identification, and goals are written in thepatientâs care plan so that nurses as well as other health professionals caringfor the patient have access to it.
-Establishing daily goalswith the patient and family -Formulating a plan of care -Transition or Discharge planning
Implementation â Nursing careis implemented according to the care plan, so continuity of care for thepatient during hospitalization and in preparation for discharge is assured. Careis documented in the patientâs record.
-Ensures patient safety -Medication administration -Provision of ordered treatments -Patient/Family education -Provision of nursing care -Documentation in healthcare record
Evaluation â Both thepatientâs status and the effectiveness of the nursing care must be continuouslyevaluated, and the care plan modified as needed.
Care Coordination â Actionsinitiated by nurses with patients, families, and/or members of their healthcareteam to manage and correct the sequence, timing, and/or effectiveness ofpatient care from admission to discharge.

License Required: Valid current Arizona Nursing License orcompact state license and ability to become Arizona Licensed in 30 days.
Certification Required: Basic Life Support (BLS) - American Heart Associationand others as required by individual department.
Certification Preferred: Advanced Cardiac Life Support (ACLS) - American HeartAssociation, Pediatric Advanced Life Support (PALS), Neonatal ResuscitationProgram (NRP) or others as required by individual department. EDUCATION:
Required: Graduation from anaccredited School of Nursing. Continuing education requirements as definedwithin the CARE program.
Preferred: Bachelors of Science in Nursing
Required: One (1) year as an RN.
