As a RN Case Manager you need to know how to:
Coordinate care and services for patients identified as needing assistance or meeting Care Management criteria. Collect in-depth information about individual needs in order to develop a plan to meet those needs. Work with patient and family/caregivers to determine specific goals and actions based on assessment.
Coordinate discharge planning. Organizes, integrates and coordinates the necessary resources to accomplish the goals and plan.
Assess the patient’s prior level of functioning to assist the team in developing a plan of care and a discharge plan. Collaborate with Physicians and other members of the health care team on the patient’s behalf.
Identify appropriate admission and continued stay issues. Enhance the quality of patient care through effective and efficient use of resources.
Identify strategies for reducing length of stay and appropriate utilization of services.
Attend, facilitate and participate in rounds and case conferences.
Monitor efficiency and availability of services and evaluate outcomes through variance tracking, data collection.

Graduate of an accredited school of nursing, required. BSN may be required for some Care Sites.
Current valid State RN License is required.
Three (3) years of case management experience in a hospital setting is required.
