JOB TITLE: Diabetes Educator Specialist

JOB SUMMARY:

This dietitian or nurse (CDE) will work in a flex capacity, to teach Patient classes, Personal Care Home trainings, outpatient care in the Northeast region.

Diabetes Educator Specialist Serves in an advanced role providing in-depth analysis of the patient with diabetes and providing specialized care and in-depth education for persons with diabetes and their families. Serves as a team member in assessing needs, planning and providing education and care, evaluating outcomes and coordinating follow up care for persons with diabetes based on the Geisinger Scope of Practice for Diabetes Educators.

Performs duties to support the goals and objectives of the Geisinger Health System by provided the highest level of patient care and maximize patient satisfaction.

Works operationally under the general direction of the department supervisor and under the American Diabetes Association (ADA) Program Coordinator.

COMPETENCIES AND SKILLS:

Demonstrates working knowledge of diabetes management of patients with diabetes and diabetes complications.

Demonstrated flexibility in response to unexpected changes in workload, staffing and scheduling.

Demonstrates the flexibility to perform duties both independently and as a team member.

Demonstrates strong organizational, interpersonal, and computer skills.

Demonstrates the ability to effectively communicate and provide patient education.

EDUCATION AND/OR EXPERIENCE:

RN/RDN or with current Pennsylvania licensure.

Two years of professional clinical experience required.

Certified Diabetes Educator Certification (CDE) Education required HOWEVER Will accept RDN licensed applicants willing to train and pursue CDE( Certified Diabetes Educator) specialization.

Working knowledge of insulin pump therapy and Continuous Glucose Sensor required.

Salary commensurate with experience and credentials.

WORKING CONDITIONS/PHYSICAL DEMANDS:

Work is typically performed in a clinical environment.

DSME/TS Diabetes Self-Management Education, Training and Support.
