Works under the supervision of the Director, Population Management Operations and
Director, Case Management Strategic Planning.
Serves in an expanded nursing role to collaborate with GHP Gold members and Primary Care Providers. This is a dual role that provides both complex case management and health management services. Ensures safe transitions of care that will prevent readmissions and support best outcomes for patients and their families. Ensures the delivery of quality, efficient, and cost-effective healthcare services. Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the member’s health status. Works with high risk population in collaboration with providers and the health care team. Heart Failure, COPD, ESRD are targeted conditions and criteria for eligible for case management services. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the population served by Geisinger Health Plan.
Implements disease management programs for chronic condition management and ensures quality measures are met for each member
This nurse will manage a highrisk population and deliver complex case management / transitions of care services and provide disease management for members with chronic conditions.
Develops systems of care that monitor member progress and promote early intervention in acute care situations. Works effectively with other members of the health care team to optimize interventions.
Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes.

EDUCATION AND/OR EXPERIENCE:
RN license or eligible for licensure in the state of New Jersey required. Bachelor’s degree in health care preferred. Case Manager Certification preferred.
Three to five years experience in clinical nursing. Prior case management experience preferred. Knowledge of the basic concepts and principles of managed care required.
Critical thinking skills and ability to analyze complex data sets required. General computer knowledge and capability to use computers required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in an office environment. Travel required.
Orientation provided at the Geisinger Health Plan, Danville, Pennsylvania.
