Are you a Nurse Practitioner or Physician Assistant that is ready to take on an innovative role and be part of a great team? ChristianaCare is looking for a NP or PA to work in our Center of Virtual Health. The selected candidate will provide virtual continuity and comprehensive primary care to a panel of patients across the lifespan. The NP/PA will able to grow their own panel of patients and work closely with a multidisciplinary team. This practice is located at our new Avenue North location in Wilmington, DE. This is a full-time position with practice hours between 7am-8pm Monday-Friday and 8am-3pm on Saturdays. The selected candidate will rotate evening and Saturday coverage, as well as on-call between the other providers.

DUTIES AND RESPONSIBILITIES

• Assesses health status of patients through virtual physical examination and medical history.
• Analyze patient data and formulate an individualize plan of care based on patient assessment and needs.
• Evaluate and monitor the effectiveness of prescribed management plan.
• Independently review patient problems including, but not limited to medication refills, determining the need of follow up care and referring appropriately to the physician according to policy.
• Administer medical care in a safe and efficient manner consistent with regulatory and organizational policies and procedures.
• Establish priorities based on patient needs and act on these accordingly.
• Transcribe orders appropriately and within scope of practice and credentials.
• Order necessary diagnostic and therapeutic procedures within scope of practice and privileges.
• Complete appropriate and timely documentation in the electronic health record (EHR), which includes history and physical (H&P), ambulatory office notes, and message center.
• Submit billing in a timely fashion according policy.
• Utilize teaching/learning principles coupled with an expert clinical knowledge to promote and provide education and counseling to the patient and/or family including teach back.
• Participate in performance improvement, research, and/or organizational activities.

EDUCATION AND EXPERIENCE REQUIREMENTS

• Completion of a Master's degree program and board certification as a Family Nurse Practitioner or completion of a Physician Assistant program and board certification as a physician assistant.
• At least 2 years of APC experience preferred
• Experience with delivering care in a Virtual platform preferred
• BLS Certification
• License or license eligible in the states of Delaware, Maryland, New Jersey and Pennsylvania
