This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The Nephology/Rheumatology RNP (NR-RNP) will be an integral member of the Nephrology and Rheumatology Sections of the VA Palo Alto HCS. The NR-RNP will arrange laboratory monitoring and medication adjustment for patients followed by both specialties. An important focus of the NR-RNP will be coordination of care for Veterans residing at a distance from the VA Palo Alto. For nephrology patients, the NR-RNP will focus further on follow-up of Veterans who are being referred for or have undergone renal transplantation and coordinate care of these patients with the VA National Transplant Center and Portland and other outside centers. Major duties and responsibilities include, but are not limited to: Serves as the VAPAHCS renal transplant coordinator in the VA National Transplant Program (VANTP) (50%). Manages a program that includes the evaluation for organ transplantation to include kidney, kidney/pancreas, or kidney/liver transplant, education for the End Stage Renal Disease (ESRD) patients and their families, case management of the inpatient and outpatient comprehensive continuum of care involving this patient population, and coordination of care with the VANTP. Manages care of Chronic Kidney Disease (CKD) patients who are followed in renal clinics at the VA (15%). Coordinates care for End Stage Renal Disease (ESRD) patients seen outside the VA (10%). Organizes, tracks, and follows up on consults, medication prescriptions, labs, and procedures, including obtaining outside medical records when needed. These tasks involve contact with multiple sections and services across the VISN. Responsible for completion of forms, as well as communication with patients and staff via electronic mail, telephone, and secure messaging. Messages and responds or assigns to appropriate physicians.
