The Registered Nurse Care Manager (RNCM) is a PACT team member who provides comprehensive and coordinated nursing care to an assigned panel of patients in the Lafayette, Lake Charles, Jennings CBOC areas. The RNCM collaborates with both VA services and community services, as appropriate, to effectively meet the health promotion or disease prevention, acute, chronic, and long-term needs, based on the Veteran's goals and plan of care with a focus on self-management. The HBPC RNCM is a professional nurse who evaluates and analyzes care provided to patients to assure it is timely, appropriate, of high quality, and cost effective. He/she assesses, evaluates, plans, implements, and coordinates care, and confers with Home Based Primary Care Director as well as other care providers to evaluate and initiate changes to ensure quality care. As the HBPC RNCM, the nurse will provide home-based primary care, identifying problems and areas of concern, developing and initiating an individualized, patient centered, care plan including detailed documentation of all findings and recommendations, and continuous evaluation of the effectiveness of the care plan. The nurse will confer with the other members of the HBPC team, as well as other healthcare professionals in planning and initiating appropriate care. The HBPC RNCM will establish methods to monitor patients' progress, including performing injections, venipunctures and catheter care and management and maintain appropriate documentation within designated timeframes.
