This Licensed Practical Nurse position is located in the Primary Care Service at the Manchester VAMC location. This position is a full-time at 40 hours per week. This Licensed Practical Nurse, under the direct supervision of the RN, MD, DO, NP, or PA, is held accountable for delivering direct patient care to adult through geriatric aged patients. He/she recognizes the age-related needs and demonstrates knowledge and care appropriate to that age group. He/she considers the cognitive, physical, emotional, and chronological maturation needs of the patients, demonstrates competence, and correctly provides appropriate care for that age group or groups. Duties include, but are not limited to: Prepares and administers medication; oral, topical, rectal, vaginal, subcutaneous, intramuscular, intradermal, and per inhaler. Documents medication administration accurately, noting changes and reporting any deviations from normal to RN or MD/DO. Has the pharmacological knowledge to observe for desired actions, dosage, and adversities. Uses a variety of strategies to monitor for and provide pain control. Evaluates and documents effectiveness of medication. Monitors, reports, and documents IV fluid levels and site condition. Performs treatments according to established policies/procedures. Assisting in the implementation of the patient driven holistic care plan. Works effectively with others; patients, family members, professional staff, supportive personnel, members of other services and community partners. Is sensitive to the individual patient's needs and responses so that a therapeutic relationship is established. Safeguards sensitive patient information. Protects written, verbal, electronic information, and access codes from unauthorized release. Respects patient's rights, e.g., maintains privacy, confidentiality, and self-determination. Provides support for the Patient Aligned Care Team in the Primary Care Clinics following established nursing and primary care policies and procedures. Ensures that all outpatient clinic appointments are scheduled in accordance with the business rules in VHA Directive Outpatient Scheduling Processes and Procedures. Collects data and collaborates in the assessment of the health status of a patient. Implements aspects of the plan of care as delegated. Participates in health screening, clinical reminders, telephone calls and treatments as assigned. Participates in the evaluation of patient responses to interventions. Recognizes, reports, and records promptly change in physical condition and behavior of patients. Completes routine documentation of results of care and treatment according to policy, guidelines, instructions, (e.g. flow sheets) utilizing the electronic record and information network. Uses proper channels to resolve ethical issues. Differentiates between emergent and non-emergent conditions. Reports promptly and accurately records critical/emergency situations. Ability to draw, centrifuge and package lab samples when indicated. Demonstrates knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems. Demonstrates a broad working knowledge of practical nursing procedures under the general supervision of an RN or provider.
