Responsible for providing the services and support activities for the in and outpatients enrolled in Blood Conservation Program. Utilizes medical knowledge, nursing education and experience to assess and interpret clinical data required to design, evaluate, coordinate and implement a plan of care appropriate to adult, geriatric, and pediatric patients.
Completes Daily Inpatient Rounds to review all aspects of each patient's treatment plan, lab results, current medications, and current status related to their blood conservation program treatment regimen and outcomes.
Meets with physicians, residents and fellows, Physician Assistants, and nursing staff, case coordinators, laboratory staff, patients and their family members to provide education and advice regarding blood conservation program treatment plans, lab values, medications, and expected outcomes.
Provides one-on-one patient and family teaching and counseling regarding all aspects of the blood conservation program and the patient's individual treatment plan based on system guidelines.
Acts as a resource and liaison between patients, family members, physicians, residents, physician assistants, and nursing staff to ensure that questions are answered and patients and family members are apprised of options.
Receives and responds to incoming requests for information and evaluation of particular patients.
Discusses specific patient information with providers, nursing staff, and laboratory staff providing guidance to them regarding various treatment options and plans, advice on medication dosing, appropriateness or options to transfusions, interpretation of lab values, clarification of orders, etc.
Receives, triages, and determines appropriateness of incoming patient referrals, and makes alternate treatment recommendations, as required.
Maintains and provides accurate information regarding participating physicians, their specialties, office locations, and office hours to provide to inquiring patients or referring providers, as requested.
Refers questions to the Physician Director, as necessary.
Coordinate the work flow, monitor the daily patient care and consultations, and track incoming consultation requests for prioritizing work assignments.
Receives and answers all pages and calls regarding patients from internal and external providers, nursing staff, other clinical support employees, community elders.

Graduate from Specialty Training Program-Nursing (Required)
