ChristianaCare is looking for an experience Clinical Nurse Specialist (CNS) to work with our HVIS team at the Christiana Hospital Newark Campus. This position is primary Monday through Friday from 8a to 4:30pm. The selected candidate will function in advanced nursing practice of Heart and Vascular interventional procedure patients pre and post procedure and take responsibility for facilitating change toward higher levels of professionalism and competency by integrating and promoting evidence based practices for Interventional Radiology and Cardiovascular patients into nursing practice.
DUTIES AND RESPONSIBILITIES:
Provides leadership by catalyzing and facilitating change toward higher levels of professionalism and competency.
Manages care of the Heart and Vascular Interventional patient within agreed upon protocols by HVIS physicians and Nursing pre and post procedure.
Acts as a clinical consultant and resource to patients, family, and staff in area of specialty to include Heart and Vascular Interventional Procedures.
Facilitates continuity of care for complex patients
Collaborates with professionals from multiple disciplines to enhance patient outcomes.
Utilizes teaching/learning principles coupled with an expert clinical knowledge to promote and provide education to the patient's family.
Tracks patient outcomes and relevant data throughout the system using either automated or manual systems.
Appropriately utilizes advanced clinical decision-making skills.
Provides formal and informal in-services in which in depth knowledge base is shared with staff.
Collaborates in the review of educational materials.
Assists staff in the development of innovative and cost-effective patient programs of care.
Utilizes, participates in, and disseminates research to enhance practice.
Promotes research-based practice in the provision of nursing care.
Consistently guides staff in professional development; acts as a mentor.
Promotes growth toward higher level of professionalism through role modeling.
Actively participates in Performance Improvement initiatives and provides direction.
Provides direct patient care as needed.
Consults with physicians regarding the patient's condition, diagnosis and treatment plan, medication orders and need for alternate dispositions if necessary.
Initiates referrals to appropriate resources and services, promoting a collaborative approach to patient care.
Initiates pre-printed order sets and clinical protocols according to patient diagnosis as indicated.
Writes orders and documents in the medical record as indicated;
Initiates requests for laboratory data and radiologic studies as needed and interprets results as related to the treatment plan.
Informs, counsels and instructs individuals and families on their specific health problems and test results.
Prepares and maintains complete and accurate medical records and other documents.
Attends, participates in discharge planning and multi-disciplinary case discussions and/or family meetings to promote high quality of care as necessary
EDUCATION AND EXPERIENCE REQUIREMENTS:
Graduation from Master's level clinical nurse specialist program.
At least three years of related clinical nursing experience
Licensure or license eligible for Registered Nurse and Advanced Practice
ACLS Certification
Advanced practice certification in specialty is preferred
