The LVN is responsible for providing direct patient care to and assisting with care coordination of Primary Care patients within Patient Aligned Care Teams (PACTs), supporting services and clinics in Chula Vista, California. PACT clinics potentially operate between the hours of 0630 and 1800 Monday thru Saturday, requiring adequate nursing support. Basic Requirements: Citizenship. United States Citizenship English Language. Proficiency in written and spoken English. Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
