St. Peter's Health Partners is seeking an experienced Nurse Practitioner or Physician Assistant to join its established PACE program located in Rotterdam, New York.

Trinity Health is the largest provider of PACE (Program of All-Inclusive Care for the Elderly) services in the country with 13 PACE organizations operating in nine states.  PACE is designed to provide the entire continuum of care and services to frail seniors, helping them to maintain their independence in their home and community for as long as possible. This opportunity would be for coverage at the Rotterdam PACE Center. 

One to three years of NP or PA experience with an elderly population is required. Interested candidates should be Board Certified and hold a State of New York license and DEA.
