The Manager, Case Management, assists the Director in managing the daily departmental operations of the Case Management department with the goal of maintaining adequate staffing levels, maintenance of departmental quality metrics, efficient workflow, and patient throughput. Acts as a resource and mentor to all staff.

RN, 3-5 years clinical experience required with prior discharge planning and/or utilization experience preferred.

Responsibilities:
-Provides orientation and mentorship to new and existing staff members
-Works with Director to identify and facilitate learning experiences for staff, including CEU opportunities -Collaborates with Director in the hiring process for new staff
-Collaborates with Director on performance improvement initiatives .
-Performs chart audits to ensure compliance metrics are achieved
-Assists Director with investigations of patient issues/complaints
-Participates in interdisciplinary committees engaged in patient care improvement
-Manages a limited caseload and serves as a resource to other SW/discharge planners
-Manages PTS requests and day to day staffing needs of SW/DCP staff
-Stays abreast of case management and discharge planning best practices and staff utilization of same
-May be called upon to cover cases for discharge planning if departmental needs warrant
