The Registered Nurse - Residential Rehabilitation Treatment Program (RRTP) Admission Coordinator serves as the subject manner expert in CLC and MHRRTP screening and placement processes and has a keen understanding of the admission criteria of each CLC and MH program area. The Admission Coordinator works in collaboration with the integrated healthcare system to provide a seamless, timely referral system to Veterans seeking admission to the CLC and MH programs. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The RRTP Admission Coordinator works closely with the interdisciplinary treatment team for the Veteran admission screening process and is an integrated part of this team. The RRTP Admission Coordinator arranges referrals or services on behalf of the Veteran to assure that necessary and appropriate clinical services are received timely. He/she coordinates day to day functioning of the admissions program that treats a variety of clinically challenging patients requiring placement. The RRTP Admission Coordinator provides consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs including information and referral for programs, other government and/or community programs. This RN maintains a referral list of Veterans requesting admission and maintains clinical contact, reassessment of services required, and provides alternative treatment consults or contact as needed. The Registered Nurse - RRTP Admission Coordinator may assist the treatment team with discharge planning for Veterans no longer needing support or those who may require higher level of support as warranted by the individual recovery plan. The RRTP Admission Coordinator serves as an advocate for patients and is a team player to colleagues as he/she continues to enhance his/her own and the team's professional growth. The Registered Nurse - RRTP Admission Coordinator utilizes and maintains statistical information on a daily, monthly, or quarterly basis to guide decision making and maintain compliance. The incumbent must possess strong clinical assessment skills; he/she will screen and review each referral for disposition and planning. The screening and disposition are to occur by the end of the next business day if the consult is complete. The incumbent collaborates with both referring and accepting staff ensuring that Veterans are placed in the program area that can best meet their needs. The Registered Nurse RRTP Admission Coordinator will maintain a tracking tool which will guide referrals, transfers, placements and discharges. A report is given quarterly to the Long-Term Care Council reporting achievements/events/metrics/follow-up requests/needs from the Council. The incumbent collects and maintains data that will enhance program planning and placement. The RRTP Admission Coordinator will monitor bed availability through use of the bed board or daily contact with each CLC and MH charge nurse/unit clerk and/or designee.
