As a Full Time 40 hours a week Nurse Liaison for ChristianaCare HomeHealth you will identify home care needs of patients to ensure a smooth transition from acute care and post acute care settings. The Nurse Liaison supports the intake process by interacting with discharge planners, case managers and patients within the acute and post acute care settings. The successful candidate will work out of our Avenue North office in North Wilmington.
The successful candidate will engage with clinical personnel who make referrals for home health care. The Nurse Liaison will manager referrals telephonically; working 20 referrals a week and taking 20-30 phone calls a day. This person will work remotely one weekend a quarter, one holiday a year, and work in the office Monday through Friday 8am-4:30pm.
Requirements:
- RN must either possess a BSN or be willing to acquire within three years of start date.
- Knowledge of medical terminology, coordination of services and Home Health practices.
- Strong interpersonal and customer service skills
- Excellent communication and organization skills
- Ability to work and travel to hospital and post acute care settings
