SUMMARY OF POSITION:
The Licensed Practical Nurse, LPN II will provide direct and individualized nursing care to an assigned group of patients under the direction of a registered nurse of provider. The LPN II will observe, report and record medical information and nursing care provided in the electronic medical record. The LPN II will instruct patients and families according to the teaching plans. The LPN II will communicate with the healthcare team to facilitate the coordination of individualized patient care.
ESSENTIAL DUTIES- The percentage of time spent performing essential functions is 95%. Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Provide direct and individualized nursing care to an assigned group of patients under the direction of a registered nurse/physician according to the Pennsylvania Nurse Practice Act, and established institutional and unit policies.
Observe, report and record medical information and nursing care provided in the medical record and/or electronic medical record.
Communicate with the healthcare team to facilitate the coordination of individualized patient care.
Create staff schedule and daily assignments.
Supervise LPNs and medical assistants to include performance and provide feedback to office manager. Work with manager to ensure timely management of phone messages, emessages, forms completion, etc. and in congruence with organizational policy.
Administer medications in accordance with policy and procedures.
Instruct patients and families according to a teaching plan overseen by a Registered Nurse/Physician.
Initiate appropriate measures in emergency situations. Participate in problem-solving, planning and improvement of patient care activities including, but not limited to assisting in research, quality improvement and evidence based practice activities.
Improve and maintain professional knowledge and skills by attending and integrating new knowledge obtained through in-service, continuing education, and programs to include both written and verbal communication.
Conduct quality audits and ensure regulatory survey readiness.
Serve as a resource to clinic personnel.
Coordinate orientation and training of nurses and medical assistants.
Create and maintain clinic specific procedure manuals and patient education materials.
Participates in the interview and candidate selection process
MINIMUM QUALIFICATION(S):
High School Diploma or equivalent required.
Graduate of an approved Licensed Practical Nursing School with current licensure to practice as a licensed practical nurse by state of employment or holds a multistate LPN license through the interstate Nurse Licensure Compact or licensure eligibility required.
One (1) year of LPN experience required.
BLS Certification required.
